There is one for UK tax payers who kindly agree that FNMT can claim an additional 25% back via Gift Aid. And there is a second form for overseas and non-UK tax payers. Please ensure you complete the correct form. Select by choosing the correct link below and filling in the form.

When you submit, if you are able and choose to allow us to claim Gift Aid we will receive your confirmation that you are happy for us to claim 25% tax relief from HM government on your kind donation.

Either way, our team will then email you to confirm receipt and welcome you to the First Note Music trust donor group!
